چکیده

T here was hope that endomyocardial biopsy would provide pathogenic information leading to subclassification and specific treatment of idiopathic dilated cardiomyopathy. This is an objective that has not been met. Achievement of this goal will clearly require the pathologist to move beyond the standard tools of light and electron microscopy to new and emerging techniques that go beyond visual interpretation of tissue and cellular morphology. The once-novel act of safely obtaining human myocardial biopsy specimens nonsurgically, then within the purview of only a few experts, is now trifling. It is what is done with the tissue that counts now.

Clinical merit of endomyocardial biopsy.

At this time, endomyocardial biopsy has proven validity as a diagnostic method in few circumstances. However, it is overused. In the near term, the extent of its use should be modified by knowledge of its therapeutic relevance in patients with myocarditis. In the long term, numerous new techniques for studying pathophysiology at the subcellular and molecular levels will demand a central role fo...

A Model of Myocarditis in Humans Jay

In this issue of Circulation, Midei and colleagues1 from The Johns Hopkins University report a high incidence of myocarditis in women with postpartum congestive heart failure. Myocarditis was first convincingly documented to be a cause of peripartum cardiomyopathy in 1982 by Melvin et a12 at King's College Hospital in London through use of premortem transvenous endomyocardial biopsy. Since then...

Incidental extraction of a temporary epicardial pacemaker wire with right ventricular perforation during endomyocardial biopsy: a case report.

INTRODUCTION Right ventricular perforation during endomyocardial biopsy is an unusual, although potentially life-threatening, complication caused with the tip of the bioptome. The majority of perforations in heart transplant patients can be managed without surgery owing to adhesions nearly obliterating pericardial space. CASE REPORT We report a case of heart transplant patient who suffered ri...

Endomyocardial Biopsy: A Clinical Research Tool and a Useful Diagnostic Method

The routine indication of endomyocardial biopsy (EMB) in myocarditis has long been a matter of debate [1]. Although always claimed as the ultimate diagnostic tool for myocarditis, its low sensitivity, low availability, high cost, and the inherent risks of an invasive procedure have led many physicians to avoid performing it. Yet, at present EMB continues to be the “gold standard” for the diagno...

Value of endomyocardial biopsy in infants, children and adolescents with dilated or hypertrophic cardiomyopathy and myocarditis.

Endomyocardial biopsy was performed in 20 symptomatic pediatric patients with the following clinical diagnoses: dilated cardiomyopathy in 16, hypertrophic cardiomyopathy in 2 and myocarditis in 2. Transfemoral biopsy was performed without complications in patients aged 2 months to 16 years; 6 were less than 1 year old. An average of five right ventricular specimens were obtained for histologic ...
